To make a referral Call the SOTM office at 972-709-1180. Our staff will request some initial information and promptly schedule a no cost mental health assessment.  You can also fax info to 844-273-3217 or contact us via email: at info@sotmlifeskills.com. All initial screenings are complimentary. Ask your Doctor: Just as your physician can write a prescription for medication, they too can submit a prescription for individual, family or group therapy as a requirement for your treatment and recovery plan. This prescription notice will help with insurance reimbursement.
